Electronic Individual Pledge Card:  This version of the pledge card allows you make your pledge and submit online. The pledge card is a downloadable PDF form. When you have downloaded the PDF, fill in the blanks electronically and hit the submit button. Your completed form will be emailed to Four Bands.

Downloadable/Printable Pledge Card: This version of the pledge card allows you to download the PDF file and print the form. Complete the form and return to Four Bands by dropping it off or mailing it to 101 S. Main St., in Eagle Butte.

Pledge to WIN! Anyone completing a pledge card from September 1st to December 18th is automatically entered into a drawing for one of three gift baskets that will be awarded in December. The gift baskets will contain items from area businesses and organizations.
